Providers and medical staff can send patients to Moffitt for care by filling out and submitting our Online Referral Form. No login is required. Upon submission of the form, the referral confirmation page will provide the fax number to send corresponding patient medical records.

Does Moffitt Cancer Center have an emergency room?
Moffitt does not have an emergency room, but takes patient appointments Monday through Friday from 7am-8pm. Emergencies that occur in the hospital are addressed, of course, but for outside emergencies, please call 911 or go to the nearest emergency room.
